FW: New rounding structure


Below is the proposed new rounding structure. We are trying to improve efficiency, decrease redundant patient visits and rounds, and improve teaching on the in-patient service. We were going to move up sign-out time to 7:15 AM, to make it universal across both services. We are hoping this structure will help to get orders/discharges done in a more timely manner by staffing earlier while still allowing intern autonomy. We are hoping to implement this starting 10/15/18. Please let me know if you have any questions or concerns. I will discuss more with faculty at the upcoming faculty meeting.

Proposed rounding structure: (Pre-rounding style)

7:15-7:20: brief morning teaching by overnight senior
7:20-7:45: sign-out
  • Keep this brief overnight events and to-do’s (will work on I-PASS format, see attached)
  • Meant to be resident-resident sign-out, will discuss at faculty meeting about faculty involvement
7:45-9:00: computer round on patients and interns see their patients individually
  • If interns don’t think they will be able to complete this in the given time frame, may need to come in earlier to start looking up patients
9:00: interns meet with attending and senior to group round on patients
  • Brief SOAP presentation of patient, go in to patient room as group, staff immediately after with updated plan and place orders, brief bedside teaching
  • Team A with 2 interns
    • While one intern seeing/discussing their patient, other intern working on note/orders/consults/etc for pt already discussed
  • Goal 8-10 min/pt
10:15- Team B meet for interdisciplinary rounds
  • Senior and attending go to these, interns go if able
10:30- Team A meet for interdisciplinary rounds
  • Senior and attending go to these, interns go if able
Rest of morning finish up notes/to-do’s before afternoon sign-out
