Inpatient updates

Medicine:
Code blue/Rapid response in PT and cardiopulmonary rehab starting 10/22
We will respond to all of these starting 10/22. There are code carts located in each location. See attached map for locations of these areas.
Mock codes and rapid responses starting 10/22
We will be starting to run more drills. Please treat these like true rapid responses and codes, so we can continue to build our skills and monitor response times and team roles.
Advanced Directive Documentation
Try to use the “Advance Directives” tab in both the inpatient/outpatient setting when recording patient’s decision making capacity, surrogate decision maker, and advanced directives. Chaplains can also help to fill this out. I have attached screen shots from Dr. Rodden with how to document. Side note, for outpatient help with advanced directives, you can send a message to the Chaplain call pool and they will contact your patient to discuss more.
“Borderline ICU/floor patients”
For patients who are floor status, but you may anticipate needing to transfer to the ICU, try to consult critical care and discuss with them sooner if possible, so they know the patient prior to transfer and may be able to act faster if they decompensate.
Discharge If orders
If your patient is okay to discharge pending a normal test, write this in the discharge instructions (ie: discharge if echo normal)
Neurology consult order for stroke alert patients
Please check “consult neurology” box for all stroke/TIA patients you are admitting with the stroke order set. This will ensure the patient is added to the neurology list to be seen the next day.
Kayexalate shortage
Kayexalate is on shortage. Valtessa is an alternative for hyperkalemia and has less of a risk of colonic necrosis.
Fluoroscopy machine up and running
The fluoro room is up and functioning now, so no need to send any diagnostic imaging to SAH.
Antibiotogram available in epic
The 2017 sensitivity data for each hospital is available in epic. Got to the dashboards and click “antimicrobial stewardship”.
The new booklets just got approved and we should have them soon.
OB:
Delivery table count with nurses
Nurses will be doing a pre and post-delivery table count with the physicians for all patients. This is to increase compliance with joint commission requirements.
