Pod Meeting Agenda – October – Thankful Thursday of Staff Appreciation Week!

 

ICE CREAM SOCIAL TODAY- woo hoo!

·      Introductions/ice breaker –  What is one thing you are grateful for this week?

 

·       Quality data by Pod, Trophies for 4 quality measures!!

      • Data from September to review (attached)
      • Winners? (see separate email)

 

 

·      Plan for addressing all 4 quality measures

o  Who What When’s for each quality measure –

      • Mammograms – current incentive for MAs/gift basket for patients
      • Colon cancer
      • HTN control
      • Diabetic control

 

·      Research and QI projects –  

    • QI projects – Update team on plans - residents have chosen one of the 4 quality measures, see below with baseline rates from June and goals. 
        • 84th Diabetes/A1C: 59.6% (goal 71%)
        • Thornton Colon cancer screening: 54.7% (goal 71%) – REVISED GOAL IS 60% for this fiscal year
        • Blue Diabetes/A1C: 70% (goal 71%)
        • Purple Breast cancer screening: 71% (goal 80%)
        • Orange Diabetes/A1C: 62% (goal 71%)
    • Research Projects – updates on topics/plans

 

·      Clinic issues in Process–

    • Updates on past issues (can refer to minutes from last meeting)
    • Review Clinic Updates

        -Pap ordering – can use universal order (automatic reflexing), other orders work as well

  • LGBTQ education update –

Trans health conference October 27th

Each pod should identify at last one person interested in helping with the educational plan for pods and call center (i.e. lunch and learn or time in pod meetings) – no prior knowledge needed, just an interest in helping!  Please email Beth G with names

  • Suboxone Update –

                Inductions have started at 144th

Computer screens in the pods will have a laminated reference card with Suboxone for providers/phone numbers

Any other pod-specific updates?

 

·      New clinic/pod issues to discuss

 

    • Discussion of use of timers to help patient wait times
    • Medication lists for patients – can give patients the printed copy of their med list to keep in their wallet, review process (idea from last week’s M and M analysis).  Other ideas on this?
    • Snacks for Pod Meeting – each pod can discuss and decide about this.  Can consider a sign-up sheet to rotate who brings snacks?

 

    • Any other recent clinic issues or challenging situations to discuss?

RE: September Book Club 9/10

  Hello!   So, this week is looking a bit chilly.  To make sure to enjoy the outdoors before it gets cold and we have to zoom all winter, I think we should move book club to Thursday, 9/17.  I hope everyone can still make it (and, you have an extra week to read the book!)    As a reminder, we will be discussing So You Want to Talk About Race - Ijeowa Ilua.      You have some more time to vote on location/food; please let me know if you haven’t voted yet, or would like to change your vote.   Anthem Park, just north of the hospital (where we had the last book club), with food from Ras Kassa’s Ethiopian restaurant in Lafayette.
