Transitions of Care Management (TCM) Visit Update: If a provider calls a patient who has been hospitalized within 2 business days can they be billed as a TCM visit when they come to clinic?  The answer is YES, see details below :)

 

Tickling (😊): Please consider a using reminder or tickler system for tracking urgent or serious lab/imaging orders - the "track order" button only goes to the overdue order queue that MAs review, not to provider's overdue orders.  You can use Epic to create a reminder or try another process that works for you.

MAs have been/will be tracking paps, pathology samples and 2nd newborn screens.

 

Flow Master Cisco Phones: An MA is carrying this phone daily for urgent issues that arise, here are the numbers-

Cisco phone Thornton – 303-925-4232

 

Don't send a referral order for Psychiatry in Epic - The referrals team cannot send these anywhere as psych is usually a different service covered by a different insurance benefit.  The Integrated BHP team also can't process these referrals – they usually just call the pt and have them call their insurance.  Typically mental health benefits are listed on the back of the insurance card.  Best is to have BHP meet with pt in person if available.  Second choice is to message the Integrated/BH Care Team rather than place a referral.

 

GC/CT test prior to IUD placement -  This is usually done prior to IUD placement but can be done at the time of placement. Please let patients know whenever you are sending a test so they realize it may show up on their visit charges.  Some patients have been surprised to find they are charged for this lab test.  You should add the appropriate ICD10 code as well.

The time frame for the call is 2 business days after discharge. If your contact with the patient takes place more than 2 business days after discharge, you cannot bill as TCM

Things to discuss are:

1.       Does the patient know and understand their discharge diagnosis?

2.       Any questions about discharge instructions?

3.       Any new symptoms or health issues since discharge?

4.       Do a medication reconciliation- check medication access

5.       Check that they connected with recommended community resources, home care services, specialist appointments

6.       Advise them to schedule a follow up within 14 days of discharge date but ideally within 7 days - route a message to the call center and integrated care team to facilitate this as below

Documentation: 

Open a telephone encounter and select "Transition of Care" as the call reason

Use the smart-phrase .TCMCALL

Add a short narrative

Route the message to the Integrated Care Team (and the call center if an appointment is needed)
