Pod Meeting Agenda – February 2019

 

• Circulate Sign-In sheet and scan/email/give to Jenn Stradley
• Assign someone to take Minutes (can use the agenda as a template) and email to your Pod/Karen Martin/Beth Groff

 

• Introductions/ice breaker –  Moment of gratitude – what are you grateful for today?

 

·       Quality data by Pod, Trophies for 4 quality measures!!

• Most recent data from January to review – see attached, includes Thornton!
• Winners from your pod/clinic for all four quality measures

 

·      Plan for addressing all 4 quality measures

o  Who What When's for each quality measure –

• Mammograms –
• MAs should add data from pt agenda sheets
• Make sure we are adding external results to HM
• Colon cancer – Blue t-shirts coming to wear in March – "Ask me why I am wearing blue"
• MAs should add data from pt agenda sheets –  getting report ideal but not necessary for Centura
• HTN control
• Diabetic control – If a patient has had an A1c done by an endocrinologist that can be added through the HM Results Console
• Press Ganey – see below

 

·      Research and QI projects –  Submitted to Research Forum this week!

 

• QI projects – Update team on current plans, see below with baseline rates from June and goals. 
• 84th Diabetes/A1C: 59.6% (goal 71%)
• Thornton Colon cancer screening: 54.7% (goal 71%) – Revised goal is 60% for this fiscal year
• Blue Diabetes/A1C: 70% (goal 71%)
• Purple Breast cancer screening: 71% (goal 80%)
• Orange Diabetes/A1C: 62% (goal 71%)

·      Clinic issues in Process–

• Updates on past issues and clinic updates (can refer to minutes from last meeting)

• Press Ganey visits to our clinics –
• We are still waiting for report/recommendations
• Remember that we are scored on our excellent/very good responses (=Top Box) but overall most patients are still generally happy 😊 We can use more responses to prove that better.
• We'd like to focus more on increasing return rate - brainstorm ideas for your pod
• Ideas - Providers telling patients we'd like their feedback at the end of the visit? Can consider adding dot phrase to AVS requesting feedback
• Other ideas?
• Very soon patients will start to get surveys via text message in addition to email or mail
• Tracking Important Orders in Epic –
• For urgent/serious orders, providers should use the reminder function in Epic or another tickler system
• Track Order button sends alert to MA queue not provider
• MAs are already/will be tracking paps, pathology, second newborn screens
• Provider/MA team may want to work together on this
• 84^th clinic – Presentation on Care of Transgender Patients in our clinic by Beth Buehrer/Siera Scales 
• Plan to roll this education out to other clinic sites in coming months
