POD MEETINGS ARE THURSDAY from 4-5pm.
(3rd Years - Reminder to clarify who is leading meeting this week in your pod)
Pod Meeting Agenda Template – April 2019 (modify as you’d like!)
· Circulate Sign-In sheet and scan/email/give to Jenn Stradley
· Assign someone to take Minutes (can use the agenda as a template) and email to your Pod/Karen Martin/Beth Groff
· Introductions/Moment of Gratitude – what are you grateful for this week?
· Quality data by Pod, Trophies for 4 quality measures!!
· Most recent data from March – coming in separate email from Karen
· Winners from your pod/clinic for all four quality measures
· Plan for addressing all 4 quality measures
· Mammograms –
· Numbers dropped for all pods due to a reporting issue –
- Prior to the correction mammogram orders were counting as completed mammograms and some older mammo results were counted as well
- Good news - Mammos done in the two years before age 50 now pull through to show pts are up to date (was not the case previously)
· MAs can add data from agenda sheets. Any other new ideas?
· Colon cancer – Blue t-shirts “Ask me why I am wearing blue”- wearing on Wednesdays, how did this go? Should we do it again?
· MAs should add data from pt agenda sheets – getting report ideal but not necessary for Centura
· HTN control – Rechecking BPs at end of visit, scheduling f/u if not at goal
· Diabetic control – If a patient has an A1c done by an endocrinologist the value can be added through the HM Results Console
· Diabetic clinics at 84th, now starting at 144th
· New monetary incentives for MAs for quality measures are happening! (Gina to discuss at 84th)
· Question on Pre-Visit Planning-
After doing their Pre-visit planning how do MAs communicate screenings that are due to the provider?
Is communication happening during huddling time?
Other strategies that are working? (MAs bringing up screenings with pts during rooming, pending orders, etc?)
· Research and QI projects – Research Forum coming in May!
· QI projects – Update team on results of projects, see below with baseline rates from June and goals.
· 84th Diabetes/A1C: 59.6% (goal 71%)
· Thornton Colon cancer screening: 54.7% (goal 71%) – Revised goal is 60% for this fiscal year
· Blue Diabetes/A1C: 70% (goal 71%)
· Purple Breast cancer screening: 71% (goal 80%)
· Orange Diabetes/A1C: 62% (goal 71%)
· Research Projects – updates on plans/results
· Clinic issues in Process–
