Pod Meeting Agenda for Tomorrow May 23rd

POD MEETINGS ARE THURSDAY from 4-5pm.

(3rd Years - Reminder to clarify who is leading meeting this week in your pod)

 

Pod Meeting Agenda  – May 2019

 

·         Circulate Sign-In sheet and scan/email/give to Jenn Stradley

·         Assign someone to take Minutes (can use the agenda as a template) and email to your Pod/Karen Martin/Beth Groff

 

·         Introductions/ice breaker –  Moment of gratitude – what are you grateful for this week?

 

·       Quality data by Pod, Trophies for 4 quality measures!!

·         Most recent data from April – attached

·         Winners from your pod/clinic for all four quality measures

 

·      Plan for addressing all 4 quality measures

·         Quality for this fiscal year is tallied at the end of June - now is a great time to review non-adherent lists! (You can review where to find this in Epic if needed)

·         Mammograms –

·         Mammo availability is temporarily delayed (and no longer available at 84th) but a new radiologist is joining soon which should help

·         New ideas?

·         Colon cancer –

·         MAs should add data from pt agenda sheets –  getting report ideal but not necessary for Centura

·         New ideas?

·         HTN control –

·         Rechecking BPs at end of visit, scheduling f/u if not at goal

·         New ideas?

·         Diabetic control –

·         Diabetic clinics are now available at both 84thand 144th - remember to refer patients and encourage them to keep the appointment!

·         Press Ganey

·         Survey will be shorter as of 7/1. Text message link coming soon

·         Continue to encourage survey completion, new ideas?

 

 

·      Research and QI projects –  Research Forum went well - 84th diabetic specialty clinic project won first place in the Shark Tank, congratulations!

 

·         QI projects –

·         Pods will be choosing new projects for the next academic year in the coming months, be thinking about what would make a good project.  Any ideas to float?

·         Second year residents take the lead on this but we are hoping for more staff and faculty involvement this next year

 

·      Clinic issues in Process–

Updates on past issues and clinic updates

  • Can refer to minutes from last meeting
  • Behavioral health will be shorter staffed in coming months –
  • SW interns leaving for the summer, Erica at 144th is stretched thin with Suboxone project as well.
  • Still try to get someone from ICT to see pt in clinic when possible, avoiding an in-basket message which is harder to manage

 

·      New clinic/pod issues to discuss

 

·         All clinics - Any other recent clinic issues or challenging situations to discuss?

·         84th clinic – Presentation  on HCC coding from PHP

·         Thornton - Discussion on transgender health care with 84th guests
