July Clinic Update - with updated Well Child Checklist attachment
Clinic Updates July 2019 –
Resending this ATTACHING UPDATED WELL CHILD CHECKLIST w current vaccine names. Let me know of any issues or corrections!
Some of these topics should have been covered at your Pod Meeting last week but some are new, enjoy 😊
Durable Medical Equipment and Oxygen Order clarification – (Wheelchairs, walkers, etc) Please refer to the back of the form for information needed, interns please ask preceptors/faculty for help if needed!
Reminder that Denver Health Medicaid patients can't be seen in our office (only can be seen at Denver Health)
Clinic Teambuilding Event for all 3 clinics!- Coming August 29th from 4-6pm, will be offsite with dinner, clinic closing earlier, watch for more info!
Immunization orders - Improving our process to avoid vaccine errors – Provider should review and sign CIIS forms and place all vaccine orders. Interns need sign off from Preceptor. Try the SmartSet "Pediatric Immunizations by Age" See full details below.
Depoprovera - Patients will be given a window from 11-13 weeks for getting injection. Per CDC, Depoprovera is still effective up to 15 weeks. So if a patient is more than 2 weeks late they will need a provider visit w pregnancy test to restart. If you see a patient on Depo and address it in your note they won't need a separate annual visit in order to get Depo.
Referrals to DM educators/nutrition: need to mostly be for just diabetes/prediabetes, they are getting a lot of referrals for GI problems and some peds referrals-- many of these they will not be able to help.
Convenient Care Clinic – Reminder that these appointments are for urgent issues primarily, not management of chronic health issues or refills on controlled substances.
More Detail for Immunization Orders - To avoid vaccine errors –
- All CIIS forms must be signed and dated by the provider and the MA before a vaccine is given.
- Providers must always place the vaccine order, before the vaccines are given.
- All pods and precepting areas will have copies of current IZ schedules
- If you use the Smart Set for the WCC Fastlane, "Pediatric Immunizations by Age" or "General Immunizations" you will be prompted on the correct orders 😊 ****see screenshot below****
- Provider should document the vaccines to be given in the note, if the vaccines are being ordered for a future date.
- Interns must also have the preceptor sign and date the CIIS form that indicates which vaccines are to be given.
- In addition to signing and dating the CIIS form, the MA must document the patient's insurance on the form and place the vaccine stickers on the form. Polio is an exception to this rule. The patient's demographic sheet must be attached to the CIIS form after scanning and the completed form must be placed in the designated area for the VFC vaccine coordinator to review.
