Pod Meeting Agenda  – August 2019

 

·         Circulate Sign-In sheet and scan/email/give to Jenn Stradley

·         Assign someone to take Minutes (can use the agenda as a template) and email to your Pod/Karen Martin/Beth Groff

 

·         Introductions/ice breaker –  your choice

 

·       Quality data by Pod, Trophies for 4 quality measures!!

·         Most recent data from July – possibly will be coming in separate email

·         Winners from your pod/clinic for all four quality measures (if we get the data)

 

·      Plan for addressing all 4 quality measures

·         Mammograms –

·         Breast Cancer Awareness month is October – we will likely do pink shirts again.

·         Any other ideas for October?

·         Colon cancer –

·         We are double checking that reports are being linked and resulted correctly with our current process.  If you notice errors on your non-adherent list (colonoscopy in HM but pt still showing as non-adherent) give Karen that info

·         New ideas?

·         HTN control –

·         Rechecking BPs at end of visit, scheduling f/u if not at goal

·         Diabetic control –

          •  Diabetic clinics at both sites  now – how is this going? Any issues to improve?

·         Press Ganey

·         Continue to encourage survey completion, new ideas?

·         Will be working on better communication with patients when we answer the phone

 

 

·      Research and QI projects –

·         Second year residents with help from faculty - We are working with pods to choose QI projects for the coming year – any updates or discussion of project ideas?

·         All-Clinic Teambuilding Next Thursday August 29th from 4-5:30.  Family can join at 5:30 for dinner and games! - Please sign up ASAP for dinner, we need numbers! This includes providers.  If you would like a vegetarian option please note that or let Karen know.

·         New Dental Hygienist Renee Hall starting next week

·         All clinics - Any other recent clinic issues or challenging situations to discuss?

·         Lab result communication –

      • How is this working in your pod? Recent “Just Do It” on leaving phone messages. 
      • Are letters appropriate/better for some results? (Anna J at 84th)
      • What about the Portal?
      • Setting pt expectations for how/when results will be communicated at visit.

·         84th clinic –

      • Lab ordering update at 84th- on Mondays (or when Sam not available) order labs as Clinic Collect
      • Exercise equipment in chapel, sign release to access
      • Ivy Donaldson to discuss agenda-setting (?)
