POD MEETINGS ARE TODAY from 4-5pm.

(3rd Years - Reminder to clarify who is leading meeting this week in your pod)

 

Pod Meeting Agenda  – December 2019

 

·         Circulate Sign-In sheet and scan/email/give to Jenn Stradley

·         Assign someone to take Minutes (can use the agenda as a template) and email to your Pod/Chrisaba Corfield/Beth Groff

 

·         Introductions/ice breaker –  Moment of Gratitude - what are you grateful for today?

 

·       Quality Data

·         Data may come in separate email if available

·         Winners from your pod/clinic for all four quality measures if available

 

·      Plan for addressing all 4 quality measures

·         Mammograms

·         Patients getting reminder calls/letters through Centura.  New ideas?

·         Colon cancer

·         FIT, Cologuard or Colonoscopy process issues?  New ideas?

·         HTN control

·         Rechecking BPs at end of visit, scheduling f/u if not at goal.  New ideas?

·         Diabetic control

·         Diabetic clinics are at both 84thand 144th - any improvements needed?

·         Press Ganey - any updates

 

·      Research and QI projects –   

·         QI projects –  Led by 2nd year residents and faculty - Update on your pod's QI project, anything to work on as a team?

·         Research projects - Update on research projects from 3rd year residents

 

·      Clinic issues in Process–

Updates on past issues and clinic updates (can refer to minutes from last meeting)

§  Communication of results (at 84th) - How is this going

 

·      New clinic/pod issues to discuss

 

·         All clinics - 

§  Patient flow -

·         Updating board when providers are running behind - helpful for MAs to communicate with front desk so patients can be notified

·         Team is working on ways to streamline check-in, scheduling appts correctly helps as well

 

§  Discuss how to communicate when providers are out of the office and who is covering for them

·         144th has had a white board for providers to mark when they are out - is this still a good process?

·         At Thornton and 84th paper mailboxes have been marked with a yellow card and who is covering - is this happening and should it?

·         Arenet used to send an email with coverage - would this be helpful?  Providers would need to let Alberta know reliably if so

 

§  Epic Messages

·         Still don't use staff messages and routing comments - nothing saves in chart

·         Residents - when you message the call center still/again/please let them know the diagnosis for which the pt needs to be seen, can specify 20 or 40 min visit as wellDesk space in pods can still be at a premium, sometimes you may be asked to shift to a different seat, flexibility is much appreciated

§  Prior authorization for meds - Providers can give paperwork back to your MA to take care of rather than giving to medical records/placing to fax.  This allows MAs to follow through on the PA process

§  Vaccine orders - Providers, use Smart-Set for vaccines, charges are automatically dropped that way "General Immunizations" or "Pediatric Immunizations by Age"

 

·         Christmas Breakfast is Friday! At Perl Mack Community Center at 7AM.  Holiday Party Saturday Night at DoubleTree Hotel!

 

·         144th Clinic, 84th Clinic, Thornton Clinic -
