Circulate Sign-In sheet and scan/email/give to Jenn Stradley

Introductions/ice breaker/sharing –  Take some time for teambuilding in your pod - share a meaningful patient interaction that has stayed with you.

Mammograms, Colon cancer – Outreach Care Technician Mirela is now helping to find results of tests already done and reach out to patients who are overdue for screening.   Any other ideas or feedback on how this is going?

84th - Update on Senior Health patients? How is this going? Hope to add a Geriatrics specialist.

 

New clinic/pod issues to discuss

 

FYI beginning next Monday 3/2/2020 -

Convenient Care Clinic at 144th will be for WALK-INs for acute issues only

CCC walk-in hours are from 5-9pm weekdays (arrive by 8:30pm) and on Saturdays 8am-5pm and Sundays 10am-5pm (arrive by 4:30pm).

·         Patients can be told to come during these hours and they will be seen on a first come, first served basis

·         Exception to this is newborn follow-up visits which can still be scheduled

·         OK to still schedule patient appts from 1-5pm on weekdays

·         More to come as we work out this new process

 

 

Medicare Annual Wellness Visits –

Centura has the Express Lane workflow which we will begin using. 

This requires MAs to document depression screen and fall risk during the rooming process. (Medicare Health Risk Assessment tab)

Remember to do MIDAS reports for errors and near misses in our clinics

 

Thornton - working on not scheduling too many physicals in a given half-day, trying to schedule with PCPs

 

84th1) Patients calling for refills of controlled substances need office visit.  2) Paperwork dropped off (FMLA forms) – if not seen recently/discussed w provider may need a visit, looking at current policy for realistic timeframe to tell pts (3 -5 or more days?)  MAs can help troubleshoot if provider out of office
