RE: Pod Meeting meetings 1/23/2020

I have a patient that is deaf and we use Marty regularly.  There is not a question of “if” we can use it- just need to communicate with ED staff and let them know when, etc.  They have been fine about allowing use.

I’m currently learning sign language and have had a few deaf patients here and there in the Centura system. I have a few deaf cousins so if the patient needs to see someone, feel free to send them my way. I can ask my ASL instructor tonight about interpreters if we want to look into  a live person interpreter.  

hopefully in another 1-2 years I’ll be fluent in ASL but until then, I’m happy to help with this in any way needed.

Mammograms –  Laura Strickland

Patients getting reminder calls/letters through Centura.  Continue doing pre visit planning, give a phone number for them to schedule.

 

Colon cancer – Groff

-Give them a phone number of Rocky Mountain GI just to call.

-Are there other places we can refer to since some people cannot afford going to Rocky Mountain GI (needing to pay up front before colonoscopy?)

 

HTN control – Harmann and Liegl

Keep rechecking blood pressures when elevated. Just recheck even if provider doesn’t ask. Ensuring close follow up.

 

Diabetic control – Harmann

7 Patients at 1/22/2020 diabetes clinic. Lots of complex patients. This is scheduled as a 20 minute appointment. Can we increase time slots to 4 minute appointments? Seems like with 3 providers seeing 1 patient, makes it difficult to stay on time. Will chat with Dr. Groff to see if we can do this.

 

CGM sensors are out of stock, working on finding an alternatives. Potential cheaper alternative.

Research and QI projects –   

QI projects –  Led by 2nd year residents and faculty - Update on your pod's QI project, anything to work on as a team?

-Continuing diabetes clinic, but working on setting a schedule to make sure they see the diabetic educator in 1 month.

 

Research projects – Analyzing data for previous diabetes clinic patients to see how good we did.

New clinic/pod issues to discuss

 

Sign Language: Patient came in the other day and we were unable to provide sign language services. Lots of confusion if we can use Marty at the ED or if we had to call Purple to hire a live interpreter. Gina will talk to charge nurse in ED to discuss use of Marty and if we can use these services are not.  Will up date further.

 

All clinics - 

PHQ-9 - providers can drop the code for this - 96127.  Need to document that PHQ was performed, reviewed and addressed (dot phrase).  Previously MA's were dropping this charge but they will no longer be doing that.  A diagnosis on the problem list for doing the PHQ ensures reimbursement.

 

Diabetic Educator Referrals - We only have one diabetic educator currently (Sue!).  Please only place a Referral Order for patients with diabetes.  If issues are weight loss/nutrition/dietary issues/prediabetes these should not be referral orders, they should be an inbasket message to the Integrated Care Team. 

 

No Monday night diabetes support groups happening right now

 

Integrative Medicine Referrals - Tuesday mornings with Dr Pearson at 144th, MA's schedule this, do not send a separate order to referrals

 

Prior Auth - For Viagra and other ED meds, please just print prescriptions for patients.  When rx sent to pharmacy, MA's need to do time-consuming prior auths, these meds will always be denied.   GoodRx and generic sildenafil are other options.  Can discuss any other ways to reduce work for prior auths.

 

Nebulizers - Reminder that nebulizers are available in clinics to dispense directly to patients (though not covered by Medicare).  Otherwise can give paper script to take to med supply company or order with DME form.  MA's can instruct patients in use.

New Patients from Senior Health - Call Center can do their best to give a 40 minute slot for 1st visit but may still do 20 minutes for more urgent issues (want to keep access as well). Some new senior health patients are slipping through as follow up visits when they are not. Will get a list of senior health providers to call center so that they are aware and know these patients were seen at senior health.

 

Psych Clinic: Please keep appointments 40 minutes when scheduling these appointments.
