Important - Optional telephone visit for clinic patients

Optional process for telephone visits for patients scheduled for low-acuity visits –   This would include things like visits for elderly patients, easy med refills, depression/anxiety follow-up, mild acute illness, other visit you think could be done over the phone   - Providers are encouraged to look at your clinic schedule prior to your clinic half-day (day prior or half-day prior) - Providers have the option of identifying higher risk patients who you would recommend not physically coming to clinic and we can offer them a phone visit. -Ask the MA lead, admin MA or your MA to call patients prior to the visit so that we can offer them the option of a phone visit during their clinic visit time.              -We are working on scripting for these calls - If the patient would like a phone visit rather than coming in, the MA will mark that on the Epic schedule .    -The provider...

144th and 84th Referrals for Diabetes Clinic!

What is diabetes clinic? -A chance for your uncontrolled diabetics (A1c 9 or greater) to see a resident, diabetic educator, and pharmacist in an integrated care visit. -Patient's case will be reviewed and care plan will be developed by the diabetes clinic team. -This is a good chance for patient to learn about diabetes, learn about the medications, and learn about life style modifications they can use to control their diabetes. -Pharmacy will also follow these patients every 1-2 weeks with phone calls to help adjust insulin without patient having to come in for an appointment. -Diabetic educator will also do additional out reach over the phone -After the diabetes clinic visit, patient will be scheduled follow up back to their PCP.   Please have your MAs schedule patients into these clinics!             AM 1 st and 3 rd Mondays of the month at 144 th         ...
