POD MEETING AGENDA JUNE 2020

Introductions/check-in –  
Welcome to the New Interns!!! Please introduce yourselves to one another and let the interns know your role in the clinic :) 

Pod meetings are monthly, usually run by 3rd year residents, time to discuss/work on clinic processes, QI

 

Quarterly "All Hands Clinic Meeting" – next month

Starting in July we will have a quarterly joint pod meeting including all the pods in a single meeting via Zoom.

This will allow us to share new information, answer questions and learn from each other.   This will be in place of the regular pod meeting.

Inbox Coverage  -

Our Pods need a better system for communicating who is covering urgent issues/inbox messages when providers are out of the office

  • In the past, Alberta had a schedule for coverage on Sharepoint - using this again would allow everyone to know who is covering, would need to email her coverage
  • White boards or emails to the team could be used to communicate
  • We also can assign a "provider of the day" for each Pod to be the go-to person to help when a provider cannot be reached

Patient Portal Messages

New process is that Portal messages from patients go to the MA pool first

MAs try to address the patient's issue if possible and determine if an appointment is needed.  They are empowered to reach out to the patient/reply to their message, refill meds if able or schedule an appt if needed

If the MA is not able to help, the message is forwarded to the provider

How is this going?  Any feedback for the team on how to better deal with patient messages?

 

Medicare Wellness Visits – HRA update - (see attached rooming process)

The Health Risk Assessment is now being documented in Epic – this can be done by the patient online before the visit or the MA can ask the questions during the rooming process.  (Note- if the pt does the HRA at home, MA clicks on “File All” to import data)

We also can get the Epic questions on paper so patients can complete this in the waiting room.

We are no longer using the old paper risk assessment forms. 

More education is coming on the Medicare Wellness Visit process.
