Pod Meeting Agenda – January 2019
- Circulate Sign-In sheet and scan/email/give to Jenn Stradley
- Assign someone to take Minutes (can use the agenda as a template) and email to your Pod/Karen Martin/Beth Groff

· Quality data by Pod, Trophies for 4 quality measures!!
- Most recent data from December to review – will be sent separately
- Mammograms – 52 percent from July to 61.8 in December, good job! Working on PVP!
- Colon cancer –-Unchanged from July to December 44.3 percent
- HTN control – small drop from July to December
- Diabetic control- A1c steady. 58.7 to 59 in December
Overall seeing best improvement from Mammograms.

· Plan for addressing all 4 quality measures
o Who What When's for each quality measure – Overall not a big change when averaging all clinics. 144th still killing us though.
- Mammograms – Working on PVP
- Colon cancer – Patient "Shared Decision Making Guide" in now in BPA in Epic, can print for patient.
- Colon cancer screening T-shirts coming?
-Unchanged from July to December 44.3 percent
-Working on using the guide as above
- HTN control – Working on rechecking blood pressures. Consider rescheduling for follow up if still high.
- Diabetic control- A1c steady. 58.7 to 59 in December
· Research and QI projects –
- QI projects – Update team on current plans - residents have chosen one of the 4 quality measures, see below with baseline rates from June and goals.
- 84th Diabetes/A1C: 59.6% (goal 71%)
- Thornton Colon cancer screening: 54.7% (goal 71%) – Revised goal is 60% for this fiscal year
- Blue Diabetes/A1C: 70% (goal 71%)
- Purple Breast cancer screening: 71% (goal 80%)
- Orange Diabetes/A1C: 62% (goal 71%)
-Still doing DM clinic on 2nd and 4th weeks of the month
-Schedule under SNHC Diabetes group
-Things looks like they are getting better
- Research Projects – updates on topics/plans
-Mathias: Research on wt loss. Do pts mark this on the orange intake sheet and do providers even discuss this? Please keep orange sheets for Mathias
-Saad: Wellness and resilience study. How well can residents deal with stress and seeing how work out interventions and having a gym available helps.
-Neisler: Published article on Gay health
-Pearson: Already did one intern year
-On a side note, clinic has lost 61 lbs during their wt loss competition. Great job!
· Clinic issues in Process–
- Updates on past issues and clinic updates (can refer to minutes from last meeting)
- Flow Master phones in use for urgent issues – how is this going in your pod?
-Used to help get direct contact with our other clinics
-Seems like it has been useful

- Press Ganey will be here on Friday – Bunmi, Chrisaba, Kerrian
- Point of Care Microalbumin/Cr is now available at all 3 clinics, can order in Epic
-Yay, results in 60 seconds
- Suboxone Update –
Inductions happening at 144th, also 84th,, refer pts by messaging Erica Feinman in Epic
Any other pod-specific updates?
-Looks like things are going ok. 84th has been sending a few referrals up.
-Question if the person referring should be the one following up after or not.
- Reminder on a few December Clinic Updates
- Physical Therapy Referrals - Refer instead to Ben Fetter, our athletic trainer, for an initial consult rather than to PT. Message him in Epic.
-our PT is really backed up
-Can see attached list that Ben Fetter has reviewed and you can specify in the comments "Patient requesting: ( PT place) "
- Medication Reconciliation – If an MA discontinues a med the provider must also discontinue it in order for the change to be permanent – how are you communicating this in your pod?
