POD MEETINGS ARE THURSDAY from 4-5pm.

Pod Meeting Agenda – October 2019
· Circulate Sign-In sheet and scan/email/give to Jenn Stradley
· Assign someone to take Minutes (can use the agenda as a template) and email to your Pod/Karen Martin/Beth Groff
· Introductions/ice breaker – Your favorite Halloween treat!
· Quality data by Pod, Trophies for 4 quality measures!!
· Most recent data from September – coming in separate email
· Winners from your pod/clinic for all four quality measures
· Plan for addressing all 4 quality measures
· Mammograms –
· Breast Cancer Awareness month is October. Other new ideas?
· Colon cancer –
· FIT, Cologuard or Colonoscopy process issues? New ideas?
· HTN control –
· Rechecking BPs at end of visit, scheduling f/u if not at goal. New ideas?
· Diabetic control –
· Diabetic clinics are at both 84thand 144th - any improvements needed?
· Press Ganey - any updates
· Research and QI projects –
· QI projects – Let by 2nd year residents and faculty - Update on your pod's QI project, anything to work on as a team?

· Discontinuing medications -
· When an MA discontinues a medication it will not be removed from the med list until the provider ALSO discontinues it.
· If it is not removed completely it shows up again when the pt comes back (frustrating for our patients).
· Various pods have been trying strategies to communicate when meds need to be removed from the med list. How has this been going?
· Reminder - 30 month well child visit is standard per AAP, is already on pediatric well child document (so 24m, 30m, 36m)
· Current OB patient workflow -
· Reminder that the second visit is 40 minutes to include "preload" of patient information by the provider, there is not a separate MA visit for this now. Remember to always schedule a f/u visit before the pt leaves
· SAN experience - These are for staff and patients when they are going through a difficult time or need a lift. The experience is personalized- a gift card, flowers, meals, etc. Please think of any patients who might be good for this and pass the info on to your clinic manager or Karen
· 144th clinic -
§ Any other recent clinic issues or challenging situations to discuss?
· 84th clinic –
§ Communication of results - what is most efficient for both staff and provider? (particularly with low MA staff now)
§ What is our protocol for people who look acutely ill with physical or mental health issues at the front desk? Emily T to help lead this discussion
§ Xray process - Trying to improve this. In the meantime if xrays are not urgent one option is to offer the patient could come back another day, particularly if you anticipate a wait due to MA staffing. Of course please go ahead with same-day xrays whenever needed
