Sick visits in person - happening more, should move to seeing all children in-person, use best practices (rooming quickly, PPE)—Groff

                -Continue to schedule virtual for patients with confirmed Covid, positive home test or direct exposure

 

Referrals Update - Delay in referrals, PT, and home health- referrals out 8 days, set pt expectations, avoid duplicate referrals, use "urgent" for truly urgent ones, referral will come through the portal-- Crystal 

 

Diabetes/nutrition referral reminder – need both-- Flint

Introduce Stacey, new DM educator

 

Update on COVID vaccine-- second booster-- okay to give, can to override it in Epic-- Rachael

 

Update on PCV20—New single-dose pneumonia vaccine-- HM won't update until next Tuesday-- Rachael

 

Access Issues-- Rachael

Offer CCC appointments

R3 New Patient ticket now closed-- if still having new pts please do not reschedule same day

 

R3's start thinking about re-assigning patients-- especially high needs patients, maybe warm handoff?-- Flint
