QI and Research Project Updates
Update on Resident projects and other PDSA cycles in your pod
Patient Safety Update:
· We will have monthly updates on patient safety during this meeting
· Reminder to do Midas reports for patient safety concerns in our clinic.
Residents will review Midas reporting once a year during HPDP and also will incorporate Midas reporting into inpatient and FCM months
· Patient Safety Huddle - Clinic leads/supervisors/managers attend this on Zoom on Wednesday mornings
Any relevant updates or follow-up on patient safety in your pod?
-It is important to have follow-up/resolution on reports
Diabetic Education update
We are lucky to have more access for patients to meet with our diabetic educators!
Providers can just put in regular referral for diabetic education which will be processed through insurance
You can also message integrated care team pool or diabetic educator specifically if you have questions
MA Standard Work Reminder:
MA referral tasks include processing orders for O2, home health, sleep studies, DME, Holter monitors (sleep medicine and other referrals go to the referrals team)
-MAs do these orders since clinical knowledge is helpful to process these
-These orders do not flow into a work queue so providers should notify their MA to process these
What is the best way to notify MAs after you place orders? (tell MA after finishing visit or Epic chat or telephone message or?)
-There can be refresher trainings for MAs if needed on how to process these
Communication Discussion:
Discussion for your pod -
· What can team members do better/differently to help each other when sending Epic messages?
Examples:
Messages from patients that could be appointments - better to schedule the patient than forward these if you think an appointment is needed. This applies to both portal messages and messages from the call center.
Providers – Optional if MAs find this helpful – you can route refill requests back to MA pool rather than individuals after refilling meds – refill requests are automatically routed back to the MA who forwarded them but you can switch to Pool and click "remember recipients" (screenshot below)
Other Issues:
Please don't refill meds by ordering under a provider who is no longer here (Dr Anna Johanson, Dr Bethany Price, Dr Bruce Martin all no longer work here).
Reminder - Pulse ox part of routine vitals for adults (18+) and infants age 0-2 yrs
