St. Anthony Residency Communicaiton

Hey Providers, We are starting a diabetes clinic in hopes to bring down our A1C numbers. We will be incorporating Pharmacy and our diabetic educator in these visits. This will hopefully help with education about diabetes medications as well as diet skills needed for controlling diabetes. I believe that pharmacy is thinking about reaching out periodically to patients to titrate medications such as insulin over the phone. There has also been discussion of providing/presribing patients a continuous glucose monitor to aid in glucose control. For now, clinic will be at 84th clinic every other Wednesday. Please start having your MAs schedule patients for our first clinics in November. Vollmer scheduled in Diabetic clinic on Nov 14 th  and Aquila scheduled in Diabetic clinic on Nov 28 th . Thanks! Lauren Biehle is a pharmacist that used to work at Rose and do education for the residents, as well as, teaching pharmacist students. When Rose’s program closed, we took her on to our program. She is going to start on the in-patient service Monday 10/22 . She is not a hospital pharmacist through Centura, she is more for resident and student education.   We are hoping to integrate her and her students into the inpatient medicine service as follows. She will be available Mon-Wed mornings. She will come around during/after sign-out to look at the patient list and cases that would be good for teaching. Wed mornings, we would like to do pharmacist morning teaching (5-10min), instead of night float senior . She has a strong interest in antibiotics and anticoagulants. At our pod meeting, there were some questions that came up about using the HCM tab in epic. This is where you should make sure everything is updated so that your patients get credit for getting their items completed. In the dashboard tab of epic on the upper right hand side clinic on “link to quality measure instructional videos”. (see screen shot below). This has videos for using HCM and information about all the different quality metrics.
